We have information on services and activities that are available to help seniors and adults with physical or developmental disabilities maintain their independence, dignity and quality of life. The Aging and Disability Resource Center is a one-stop resource for individuals, their families and caregivers.
Our Information and Assistance Specialists will meet with you personally to explain Resource Center services and programs available to meet your specific needs. If the help you require isn't available from us, we'll refer you to other agencies or individuals in the community. We provide ongoing assistance to help you obtain the services you need.
What kinds of services?
Adaptive equipment: communication aids, wheelchairs, bath seats, handrails and other equipment.
Benefits counseling: help with Medicare, Social Security, Supplemental Security Income, Medical Assistance, Homestead Tax claims, Medicare supplement insurance, veterans' programs, and other public benefits.
Caregiver services: Adult Day Center, respite, caregiver resource library, informational workshops, support groups.
Chore services: snow removal, lawn and yard care, housekeeping, repairs and maintenance.
Consultation with families concerned about older or disabled relatives.
Domestic abuse referrals: physical (including sexual), emotional, or financial abuse/exploitation by a spouse of adult child.
Elder abuse referrals: financial, physical, emotional or self-neglect.
Health concerns: classes, informational presentations, screenings, support groups, exercise for the well-being of body and mind.
Home modifications: ramps, bathrooms, remodeling or design.
Housing options: public and private apartments, nursing homes, community-based residential facilities (CBRF's), adult foster care.
Nutrition: home-delivered meals and congregate meal sites in Portage County.
Personal needs assessment: professional assessment of an individual's ability to perform daily tasks either independently or with assistance. Includes an evaluation of an individual's ability to purchase needed services or referral for financial assistance. Includes options counseling and determination of eligibility for Family Care.
Senior center and community education and recreation: a wide variety of classes, activities and special events for learning and enjoyment.
Supportive home care referrals: assistance with housework, laundry, meal preparation and personal care (such as bathing and dressing).
Transportation: rides to medical appointments, Nutrition Program meal sites, grocery shopping, senior center classes/activities, and referrals to specialized and accessible transportation services.
Vocational referrals: evaluation, training, and job placement services.
Volunteer opportunities: sharing lifetimes of experience for the good of our community.
Support groups provide a wide variety of sharing and learning opportunities for older or disabled adults with specific health concerns or sensory deficits and to family members or friends experiencing the joys and challenges of caregiving.
Transition to Adult Services
Local schools, service providers, and resource centers collaborate for the purpose of transitioning students with disabilities from the academic setting to adult activities and services. The Portage County - Community on Transition (PC-CoT) meets several times annually to facilitate collaboration among all agencies. Representatives from any related group are welcome to participate, including parents or guardians and transitioning students. PC-CoT also hosts an annual Transition Expo where various organizations are available to provide information
